Cancellation Policy: Insight Wellness has a 24 hour cancellation policy. Since our services are time intensive, you are essentially paying for a particular time slot. Cancellations or rescheduling with less than 24 hours notice may result in a minimum $25 charge per one hour session. For credit and debit card customers, this charge may be assessed automatically. Anything over 1 hour will require a $40 cancellation fee.
It is the sole discretion of Mr. Cohen as to whether or not a charge may be assessed. We will make every effort to accommodate emergency situations.
Cancellation or rescheduling with less than 24 hours notice may result in use of one comparable pre-paid session, or (in the case of gift certificates) forfeiture of said gift certificate.
Late Arrival Policy: Please arrive 15 minutes before your scheduled appointment if you need to fill out any paperwork and to discuss questions and treatment options.
Late arrival may result in a shortened session. However, it is the responsibility of the client to pay for the full amount of the scheduled session. If Mr. Cohen is late in beginning your session, he will either credit your account for the time not received, or he will pro-rate your session, accordingly.
Gratuity: Tipping is appreciated. Tips may be received as cash, check, or on credit card.
Payment: Full payment must be received when services are rendered. Payment may be via personal check, cash, or credit / debit card (when available). If paying by cash, please bring exact change.
Returned Check Charges & Disputed Charges: Returned check fee is $25. This is in addition to the cost of service, and must be paid within 2 weeks. Disputed charges require formal notification, in writing, within 1 week.
By signing, you agree to pay in full for services rendered, as outlined in this form. You also agree to Mr. Cohen’s cancellation fee and policy.
